Purchase the kit that's right for you at your local RadioShack store.
Keep your receipt, you'll need to mail us a copy of it.
A) Insert the following in the package:
- 1) Completed form (included in the kit).
- 2) A copy of your receipt from the kit you purchased.
- 3) Your device.
Your repaired phone will arrive at your door in less than 72 hours if you choose express service.‡ Our screen replacement service includes a limited lifetime screen replacement warranty.
